Program Overview: What is Incident Investigation and Near-Miss Reporting?
Incident Investigation & Near-Miss Reporting is a hands-on training program for safety officers, supervisors, and operations teams. The program covers incident investigation basics, near-miss reporting systems, scene control and evidence collection, interviews and root cause analysis, and investigation reporting. Participants work across six modules over 24 hours.
Training is scenario-driven, using realistic incident scenes and witness simulations. Participants secure scenes, collect evidence, conduct interviews, and trace root causes through structured analysis. Instructors with safety investigation experience review each investigation, challenge findings with learners, and coach on report writing. The program closes with a five-hour capstone where each team runs a full investigation end to end.
Most incidents get a quick fix and a filed report, while the real causes survive to cause the next one. Near misses go unreported because nobody trusts the system. This program teaches teams to investigate properly, report without fear, and turn every incident into prevention.
The capstone is a full investigation run under realistic conditions: a staged incident scene, evidence to collect, witnesses to interview, and a root cause report to defend. Teams finish with an investigation file they can stand behind, and the habits to run the next real one the same way.
Six modules and 24 hours: investigation foundations, near-miss systems, scene and evidence work, root-cause analysis, reporting and emergency response, and a capstone investigation. Every module pairs concepts with a guided lab. It closes with a capstone where your team runs a full-cycle investigation on a mock incident for instructor review.
Delivery is hybrid across virtual and onsite sessions, scheduled around your calendar. Cohorts train as one group or in batches, with lab guides included for every participant.

Where this Training Fits in Your Team's learning path.
Incident investigation and near-miss reporting come second, after hazard assessment. Your team needs to understand risk before finding why controls failed. The path then adds root cause analysis tools and ends with human and organisational performance, so investigations look at systems and not only people.

Build Incident Investigations With Reliable Evidence and Supported Corrective Actions
Edstellar develops investigation capability through realistic scenes, reports, and witness scenarios. Participants classify events, design near-miss workflows, and document evidence before testing root-cause explanations. The final investigation is reviewed for a clear distinction between observations and assumptions, a traceable evidence record, and corrective actions that address the supported findings.
- Step 01
Consult
We explore current incident and near-miss practices, identifying gaps in classification, evidence collection, witness interviews, or ownership of corrective actions across site roles.
- Step 02
Plan
Investigation foundations and near-miss reporting lead into scene control, evidence, interviews, and root-cause analysis before report preparation and the full investigation simulation.
- Step 03
Align
We align safety-investigation expertise with EHS, supervisory, and operational responsibilities, establishing shared methods without requiring previous investigation training or specialist experience.
- Step 04
Customize
Mock scenes and fictional witness accounts reflect relevant hazards, allowing teams to adapt evidence logs and reporting forms without revisiting identifiable incidents or allegations.
- Step 05
Execute
Participants document scenes, interview witnesses, and test root-cause explanations, receiving feedback on neutral questions, evidence handling, causal reasoning, and the clarity of findings.
- Step 06
Evaluate
We assess the investigation report for traceable evidence, supported causal conclusions, appropriately qualified uncertainties, and corrective actions clearly linked to the findings and assigned responsibilities.
